We offer our Rehabilitation Respite to help ease the transition from your care to home. Taking advantage of our Rehabilitation Respite will assist individuals in high risk circumstances such as lack of support at home, interim strengthening needs, and those at risk for developing symptoms of depression. Summer Hill’s Rehabilitation Respite program provides a transitional care apartment at an affordable per-day-cost in our Assisted Living. We offer your patients and their family members peace of mind knowing that they have a trusted partner to guide them after a recent hospital stay. In addition, we are able to help coordinate necessary Medicare-certified outpatient therapies to meet your patient’s rehabilitation needs.
